Jayla and Murphy welcome Jeff Martinez, President of Brothers Redevelopment. Jeff explains that Brothers Redevelopment is a non-profit organization dedicated to providing housing and a variety of housing related services for low-income, elderly, and disabled people in Denver and across Colorado. We learn about Brothers programs including providing federally subsidized low-income apartments, a home maintenance and repair program and their information services program called Colorado Housing Connect. Meet Mary Ellen Carlow from the Senior Resource Center. She introduces us to a new collaborative program between Exempla Lutheran Healthcare Center and Senior Resource Center called A Perfect Homecoming. The program targets older adults with chronic heart failure and is designed to help them transition from the hospital to their home by providing non-medical services. The program has 3 goals; reduce readmission rates, provide cost savings for the patient, family and society and improve the patient’s overall quality of life.
